Bridges InitiativeBackgroundThe Bridges Initiative was a research project (1992-2004), funded by St Columba's Hospice, designed to extend and develop the palliative care approach to people being cared for in nursing homes. In the course of the decade up to 1999, the number of deaths in Lothian nursing homes rose from 306 to 1123. Historically, nursing homes have not been used to caring for the very sick and dying - this is now changing. The elderly population is increasing rapidly. Because a growing number of families are separated through work commitments and lifestyle, many frail elderly people are likely to be looked after in 'care homes' rather than by their own families in their own home. Recent research reveals that dementia sufferers being cared for in nursing homes often have under-treated pain problems. With two-thirds of cancer occurring in people over the age of 65 and cancer becoming a much more chronic condition, even the elderly with cancer are likely to be looked after in 'care homes'. It is increasingly important, therefore, that nursing home staff are equipped with appropriate knowledge of the palliative care approach to care for these frail/ill residents to a specified standard. The InitiativeThe St Columba's-sponsored Bridges Initiative was a five-year study undertaken by Jo Hockley, Research Fellow/Clinical Nurse Specialist at the Hospice. This 'action research' study aimed to extend the palliative care approach through direct clinical involvement with the nursing homes who volunteered to take part. A spirit of close collaboration ensured the culture of the organisation being studied was taken into account and the development of the palliative care approach was tailored to its needs. Issues were raised through interviews, focus groups and participant observation. Action plans to develop practice were discussed, agreed and then evaluated. The project is already adding to the body of knowledge of dying in nursing homes in the early 21st century.
