How to Make an Application

If you decide to apply to an advertised post please send your completed application form by post to:

Or email your completed application form to: info@stcolumbashospice.org.uk

• Completed Application Form (MS Word). Forms can be downloaded from our Application Forms page. It is a Standard Application Form, same for all positions.
• If sending by post please send the requisite number of Application Forms (please see relevant job description for details)
• You may in addition include your current Curriculum Vitae.

Should you decide to request an application pack via the telephone, post or email, the pack will be posted to you on the day we receive the request, where possible. If you do not receive the application pack within three working days, please contact us.

Please quote the reference number and job title on the outside of the envelope or in the "Subject" line of an email and ensure that your application is received by the closing date in the advert. Please sign your application form if sending by post. If sending by email you will be asked to sign your application form if you are attending an interview.
